Website Centre de Santé Univi Health Centre

·       Provide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow-ups, triage, screening, and counselling.

·       Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.

·       Participate in the development, delivery and evaluation of educational and health promotion programs.

·       Maintains accurate and timely records and encounter data in patient electronic medical record (EMR).

·       Participates in chart and case review.

·       Communicates information clearly and concisely to patients and health care providers on a regular basis.

·       Educate patients and their families about healthcare, treatments, and procedures.

·       Participate in the assessment and reassessment of patient care needs.

·       Adhere to policies and procedures as well as professional standards.

·       Other duties as assigned by the Program Director.

QUALIFICATIONS:
·       Bachelor’s Degree in Nursing, or Diploma with relevant experience.

·       Membership in good standing with the College of Nurses of Ontario.

·       Work experience in a primary care setting preferred.

·       Knowledge and ability to assess the needs of various age groups.

·       Experience in the planning, implementation and evaluation of various health-related topics in a community setting.

·       Superior problem-solving skills with the ability to prioritize and time manage in a demanding office environment.

·       Knowledge of community resources, especially in regards to health and social services.

·       Ability to work within an interdisciplinary team.

·       Current certification in Basic Cardiac Life Support (BCLS).

·       Bilingualism preferred (English and French).

·       Solid working knowledge and ability to navigate a PC windows environment, including shared drives and Microsoft applications.

·       Valid Ontario’s driver’s license and access to a vehicle, would be an asset.

WORK CONDITIONS:
·       Salary range: $65,169.10 – $81,927.00
