Job Summary
The Transitional Support Outreach Worker provides follow-up support to residents and other agency clients leaving an emergency shelter situation. The Transitional Support Outreach Worker is responsible for providing support counselling, education and information from a trauma informed approach. The Transitional Support Outreach Worker will treat clients as the experts of their own lives, provide non-judgmental information, and support individuals in the decisions they make. In addition, the Transitional Support Outreach Worker will situate individuals’ behaviours in the context of their lived experiences, provide realistic options and services to individuals and focus on reducing harms now while working to help them achieve their long- term goals.
This position involves providing emotional support, practical assistance, and outreach services to empower clients in their journey toward independence and self-sufficiency. The Transitional Support Outreach Worker will be aware of the various forms of discrimination, oppression or stress that individuals may be experiencing and act in ways that do not contribute to feelings of oppression. The Transitional Support Outreach Worker will focus on building relationships based on respect and trust.

Key Areas of Responsibilities and Associated Duties
•
Provide court support and advocacy to clients in need.
•
Provide information and refer to appropriate agencies as identified through client needs assessments and transition plans.
•
Provide support visits to the client in a safe environment of their choice.
•
Provide crisis counselling and intervention, involving the local authorities where possible.
•
Assist clients in the development of transition plans, assisting in the establishment of goals and objectives.
•
Assist clients in developing safety plans and providing referrals to assist with safe transitioning.
•
Maintain accurate and confidential client records and documentation of services provided.
•
Prepare reports and collect statistical data where required.
•
Facilitate and coordinate enrichment and support groups and workshops.
•
Provide referral to and consultation with other service providers in the community.
•
Participate on community committees that seek to promote awareness and prevention homelessness.
•
Promote the services of Maplegate/Larry’s Place and the transitional/ outreach program.
•
Participate in public speaking engagements to promote awareness of the programs and services offered.
•
Other duties as assigned.
Work Conditions

Qualifications
•
Minimum Education: Graduate of a post-graduate program in Social Sciences for equivalent from an accredited Canadian University
•
Minimum Experience: 2 years of community development experience in the area of homelessness/crisis counselling
•
Special Skills: Valid first aid and CPR certification clear vulnerable sector check, a valid Class G Driver’s license with vehicle and adequate insurance,
