Reporting Relationship

The Gichi-aya’aa Navigator will be under the supervision of the Enaagdenjged.

Position Summary:

The Gichi-aya’aa Navigator will focus on providing referral services, outreach, and advocacy to M’Chigeeng First Nation Band Member Elders (60+ or vulnerable persons).

Essential Functions

Manage an active participant (Elder’s and Vulnerable persons) caseload.
Support department elder’s leadership and community services activities.
Provide elders information on services available within the M’Chigeeng First Nation community.
Navigate elders to the appropriate supports when they require assistance with:Homemaker services
Personal care services
Light Shopping and or delivery of meals
Identify barriers and assist them in finding solutions to barrier removal.
Recognize participants’ strengths and skills and apply those attributes to appropriate activities.
Educate participants on scam awareness and fraud prevention.
Work collaboratively and communicate effectively with an inter-professional care team that may include patients, elderly families, and health care professionals.
Provide group transportation to social events or support groups.
Arrange counselling when required.
Arrange and coordinate traditional supports and/or ceremonies when required.
Provide proactive, holistic opportunities for elders, family/supporters to share their experiences and wisdom with the team for improved services and related initiatives. *Develop relationships based on respect, reciprocity, and collaboration.
Provide respite care for caregivers’ elders (when feasible).
Visit elders in nursing facilities, homes, or residential care communities.
Work one on one with case managers on referrals and outreach services.
Work with various employment entities to develop suitable worksites for elders when requested.
Submit recording and reporting time to the Enaagdenjged.
Obtain, track, and document all pertinent information in each case file electronically.
Participate in scheduled community meetings as requested.
Assist in the facilitation of elders group activities or workshops.
Work as part of a team: the team includes co-workers, partner agencies and the participant.
Attending and actively participating in team meetings and training.
Bring policy issues to the Enaagdenjged as they arise.
Maintain confidentiality according to policies, and procedures of M’Chigeeng First Nation.
Perform all related duties as assigned.
Other Duties:

Be self-directed, strategic, collaborative, flexible, organized and highly motivated and visionary.
Operate related equipment and applicable software.
Assists with communication and planning initiatives with all staff as required.
Completion of monthly, quarterly, and annual reports to the Enaagdenjged as required.
Assists with gathering data for completion of various staffing reports to the Enaagdenjged or Chief and Council when required.
Enrolls in administrative and professional development activities.
Ensure that the strictest of confidentiality and need to know level of confidentiality is always maintained.
Understands and adheres to all MFN policies and procedures.
Other duties as assigned.
Qualifications:

OSSD or equivalent.
Previous work experience in assisting elderly or as a case support in other programs.
Navigate community resources preferred, with preference given to those with knowledge of mental health and social work.
Knowledge of M’Chigeeng First Nation culture and/or community resources.
The ability to speak Anishinabemowin is an asset.
Knowledge of the Anishinabek values, culture, and traditions.

Demonstrated knowledge and understanding of the social, health, education and training and cultural needs of the M’Chigeeng First Nation community.
CRC/VSS Rationale: M’Chigeeng First Nation and its agents are in a position of trust in services to band member children and/or vulnerable adults, so a safe and secure working environment is mandatory. Applicants must provide a clear VSS Certificate and the CRC must have a record clear and/or dated of any convictions relating to sexual interference, sexual assault or related charges. The VSS must be current (30 days) and must be an original.
